IC24 has been recognised at the HSJ Patient Safety Awards alongside Norfolk and Waveney ICB and Norfolk Care Homes for our involvement in the Care Home Pilot.

The Care Home Pilot has been instrumental in addressing the barriers those most vulnerable have to accessing care at the right time. We’ve established a proactive approach by reaching out and speaking with care homes and their residents to discover any healthcare issues before they become serious, which allows us to deliver a stronger service. Utilising digital platforms enables those most at risk to access a doctor before an emergency occurs, reducing emergency department visits and reliving the strain on healthcare facilities. The Care Home Pilot has been a huge success so far as we continue to see improvements across our service by expanding the pilot to more care homes.
