IC24 and the Norfolk and Waveney Integrated Care Board have recently taken a proactive approach to providing care to those most vulnerable living in care homes. Our team noticed a specific pattern regarding contact to our 111 services and devised a system to reach those in need before urgent care is required.

This new pilot scheme has been hugely successful and lauded by the care homes participating. We have seen a reduction in the number of out-of-hours calls, relieved pressure on our emergency services and provided peace of mind to the staff members and care home residents.
