IC24 Wins Silver Partnership Award at UHUK Awards for Unscheduled Care Coordination Hub
We are delighted to announce that IC24 has received the Silver Partnership Award at the UHUK Awards for our work on the Norfolk and Waveney Unscheduled Care Coordination Hub (UCCH). This award highlights the collaborative efforts of IC24 and our system partners to enhance urgent care services across the Norfolk and Waveney region.
The UCCH brings together practitioners from different organisations, including GPs, paramedics, nurses, social care teams, and mental health specialists, to provide patients with the right care, in the right place, at the right time. Since launching in September 2023, this coordinated approach has achieved remarkable results:
- 12,291 individual 999 calls reviewed to ensure appropriate response and patient care.
- 7,640 patients diverted from ambulance dispatch to community-based care, such as the 2-hour Urgent Community Response, Virtual Ward, or GP Out of Hours.
- 62% of reviewed 999 calls avoided an ambulance dispatch, allowing faster response times for those in critical need.
- 85% of patients followed up by the UCCH had no further unplanned contact with the urgent care system, showcasing the hub’s effectiveness in long-term patient support.
This award is a testament to what we can achieve when we work together to make a meaningful impact in our community.
