I am pleased to inform you that the Kent Long Term Conditions Year of Care Programme has been "highly commended" in the annual HSJ Value in Health Care Awards held on the 22nd September.

Hazel Carpenter, Accountable Officer for South Kent Coast and Thanet CCG’s who is the SRO for the LTC Year of Care Programme has released the following message:

As the SRO for the programme I would like to congratulate you on this important recognition. The Kent Year of Care Programme is an active partnership between Kent County Council, the seven CCG’s, the four Acute Trusts, KMPT, KCHFT, SECAmb, IC24 and the Pilgrim Hospice.

The willingness to share data and embrace a new approach to commissioning has been at the heart of our success. As a result of our achievement with regard to data linkage, we are now in an excellent position to explore the potential of this approach in the context of service planning"
